Health Maintenance Guidelines Birth to 17 Years

A regularly scheduled Health Maintenance Evaluation at each of the following ages:

-1 week, then 2, 4, 6, 9, 12, 15 and 18 months
-2, 3, 4, 5, 6, 8, 10, 12, 14, 16 years
-See childhood immunization schedule ages 0 – 6 or ages 7-18

-Tuberculosis test may be required at 5 years depending on locality; optional at 9 months in areas of average risk for tuberculosis.
-Vision screening at yearly ages 3 – 5
-Hearing screening ages 4 & 5

Screening Tests

-Anemia/Lead Screening at 9 months and age 2

For sexually active females:

-Chlamydia screening -- Starting at age 15 annually

-Pap Test -- Three years after first sexual contact and every 3 years thereafter
